 Use PATCHS to submit all your medical and administrative requests


PLEASE NOTE: When submitting a prescription request through PATCHS, don't expect to be contacted. You will be able to collect the medication from your pharmacy within a few days. For any other requests, please wait to be contacted and monitor your email inbox / PATCHS account.
Please, select the appropriate request tile. If you put multiple different things (e.g.: repeat prescriptions, a health problem and an admin query) within one request, then you will get a delayed response because the request will have to be passed around to different members of our team.

Only submit a request for a clinician if you are able to have a telephone/face-to-face consultation the same day.

Why should you use PATCHS?

How to use PATCHS

  1. Register in PATCHS. Access PATCHS through the banner on the home page or by clicking here. If you don't have an account yet, click on "Register as a patient". You will need to fill in you details (name, DOB, gender, address, telephone number and email address) and set up a password.

  2. Submit your request. Select the type of request and answer 4 simple free text questions with as much detail as possible. The more information you give us, the better we will be able to help you.

    If you are submitting a prescription request, please name the medication (you can list all medications that you require within one PATCHS request)

  3. Wait for us to contact you. Please monitor your emails, as we may send a message to you. We may also give you a call during your stated available times.

    If you have submitted a prescription request, don't expect to be contacted. You will be able to collect the medication from your pharmacy within a few days


FAQS

How do I know when my prescription has been issued by the GP?

We will issue your medication within 48 hours of receiving the request. However, please note that it may take a few days for the pharmacy to prepare your prescription.

I have submitted a request and I haven't been contacted yet, do I need to do anything?

No, please wait to be contacted. Monitor your email inbox/ PATCHS account and phone calls, as we will be in contact with you through one of these channels

Can I access my medical records through PATCHS?

No, PATCHS is used to contact the surgery. You can access your medical records using the NHS App

Can I submit one request with multiple different queries?

No, you must select the appropriate request tile for each query. If you put multiple different things (e.g.: repeat prescriptions, a health problem and an admin query) within one request, then you will get a delayed response because the request will have to be passed around to different members within our team.

Can I submit a request even if it is just a question for reception?

Yes, whether you need medical advice or help with administrative queries, you can submit it through PATCHS

Do I have to register every time I want to submit a request?

No, once you are registered with PATCHS, you can log in to your account to submit a new request
